SUMMARY OF SERVICES
Based on the fact that each person admitted to the program has different
needs, the Transitions partial hospitalization program strives to restore
and promote maximum functioning capabilities and optimal health within
each patient's intellectual and cultural framework. Particular emphasis
is placed on dignity, respect and confidentiality. The program is designed
to foster independence and to improve relations with family, peers and
the community. It allows for more flexible and less restrictive treatment
by increasing levels of functioning and supporting access to progressively
lower levels of care.
Transitions is based on a 10-week group psychotherapy model. Group size
is limited up to ten (10) patients per group, and groups may be split
to increase the quality of care and the participation of each patient
in their treatment.

THERAPEUTIC SERVICES OFFERED
1. Medical Evaluation and Treatment
A licensed, credentialed physician conducts a comprehensive medical history
and physical for each patient admitted to the program. Appropriate laboratory
testing provides basic assessment data on the patient's physical status.
2. Psychiatric Assessment/Monitoring
A licensed, credentialed psychiatrist conducts a comprehensive evaluation
on every patient admitted to the program. Additionally, each attending psychiatrist
continues to monitor the patient's treatment throughout his/her stay in
the program.
3. Individual Psychotherapy
Each patient retains his/her individual psychotherapist when entering the
program. Patients in the program are followed by an attending psychiatrist
who oversees his/her treatment. The attending psychiatrist is responsible
for the primary individual psychotherapy, with the staff case coordinators
acting in a supportive, goal-directed capacity in accordance with the individual
plan.
4. Nursing Services
Licensed nursing staff provides planned, consistent nursing care in a supportive
therapeutic environment. In collaboration with the attending psychiatrist,
nursing staff participates in the planning and execution of prescribed medical
treatment and healthcare teaching.
5. Counseling Services
A qualified counseling staff provides planned, consistent clinical program
services, in a supportive and stable therapeutic environment. The services
include a family component and after-care program.
6. Psychopharmacological Assessment and Monitoring
Patients in the program may require treatment in the form of psychotropic
or neuroleptic medication. The consulting psychiatrist oversees this treatment
and the program staff assist in monitoring any positive or negative effects
on an ongoing basis.
7. Expressive Therapies
These include music, art, movement and recreational therapies. These therapies
assist patients in appropriate self-expression, increased focus and concentration,
appropriate social interaction, and are especially beneficial for the cognitively
impaired or more regressed patient.
8. Social Services/Discharge Planning
The case coordinator completes a comprehensive psychosocial assessment in
order to facilitate a discharge plan. The case coordinator develops a discharge
plan and an after-care plan for each patient. |
