Examples of Home Care Services Provided

St. Helena Hospital's Home Care Services encompasses a wide range of health and social services. They can provide medical, nursing, social or therapeutic treatment and assistance with the essential activities of daily living. Nurses, therapists, social workers and aides provide services on an intermittent basis. Here are some examples of the types of care we can provide to patients in the home:


Skilled assessment of:
Patient's physical, mental and emotional status

Environmental safety factors and need for home medical equipment    (walkers, oxygen, etc.)

Adequacy of financial resources to meet the patient's medical needs

Ability of the family to provide safe and adequate care.

Instruction in:
Health maintenance

Diabetic care

Medication management

Exercises to improve gait and maintain strength and mobility

Infection control

Nutrition

Safe use and care of medical equipment in the home

Respiratory treatments including:
Chest physio/postural drainage

Inhalation treatments

Sleep apnea studies

Tracheostomy care

Thera Pep and other pulmonary hygiene measures

Oxygen flow titration during ADL's

Nasogastric/gastrostomy feedings:
Instruction in safe food preparation, infusion and storage.

Blood draws:
To titrate or maintain a therapeutic range of medications.

For diagnostic purposes.

Ostomy care:
Assist patients in finding ostomy appliances that best meet their financial    and physical needs.

Provide support in dealing with changes in body image.

Bladder instillation and catheter care:
Change indwelling catheters or instruct in intermittent catheterization

Pain management:
Develop an individual pain management program to meet each patient's    needs.

Explore alternative treatments to control pain.

Wound care:
Provide treatment that promotes rapid healing, decreases the risk of    infection and increases comfort.

Assess wounds for early signs and symptoms of complications.

Restorative measures:
Methods to promote optimal joint function and pressure relief.

Assitive interventions and devices to compensate for neurological or    musculoskeletal disturbances so the patient's ADLs can be safely and    adequately met.

Technical support for complicated treatment regimens:
Administration of IV infusion and maintenance of IV access device

IV Lasix infusion for CHF patients

Heart Card for EKG readings from the home

Use of TENS unit for pain control

Home health aide services
(must be provided in addition to a skilled service):

Assist with bathing, medications, exercise, light housekeeping and    shopping.
